Super Tournament XXXVIII

The 38th annual MLAG State Tournament will beheld March 7-9 (Elementary and Minor) & March 14-16(Middle, Junior & Senior), 2012 at the Crown Plaza Hotel, in Grand Rapids.

The following documents are necessary to register for the tournament. Please download and return with payment by Saturday February 11^th. See notes for further information.

School Registration Form – One per school, tell us how many people you are bringing, how many teams, and determine your total cost

MJS Housing Form – Housing Form for the Middle/Junior/Senior tournament

Minor / Elementary Housing Form – Housing form for Elementary / Minor tournament

Student Health Form –A completed health form must be turned in for all students attending the tournament

Adult Health Form – Health form for all coaches, parents and chaperones attending the tournament

Roster and Health Form Cover – A sheet to list all students and adults from your school in alphabetical order. Use as a cover sheet for the competed health forms

Commuter Registration Form – Registration sheet for participants not staying in the hotel

T-Shirt Form – List what size T-shirts you need for your students and adults

Team Registration Forms – Fill out one half sheet for each team for each game. Excel version.

Extras Registration Form – For registering players that do not have a full five person team

Nomination Form – To nominate students, coaches, or parents for special awards

Tournament Schedule – Schedules for both tournaments

Sponsor Responsibilities – List of sponsor responsibilities at tournament

Sponsor Acknowledgement – All coaches must read and sign
